Tips to improve the efficacy of PDE-5 inhibitors
There are many ways to improve the efficacy of PDE-5 inhibitors (taking the pills on an empty stomach, increase the dosage, vitamin E) and I think I've just found out another one: suspend the use of the drugs for a long period of time and then resume treatment.
I didn't take any Cialis pills for a month or so and when I started taking them again results were amazing, better than ever. I remember I had this super strong erection and it didn't go away, not even after ejaculation. I was actually a bit scared and had to throw some cold water down there to lose the erection. 
I'm not an expert but maybe the body 'gets used' to these chemicals and they lose some of their efficacy. By suspending their use for some time, the body may 'forget' about these chemicals and loosen the natural defenses.

Never heard of the Vitamin E part but I sure do agree that one of the most important tips is taking it on an empty stomach. I would say it works 3 times faster than taking it on a full one

Has anybody had inconsistent luck with these pills? Sometimes they produce a hard-on like you dream about, and other times they do not work so well. It baffles me.

Depends how you take them Willie. Which one have you tired? Viagra is the strongest and works good for me on an empty stomach..

Here is another tip (obvious, but it's true): listen to the expiration date because it's no joke. I popped a Viagra as it hit its expiration date, and it did not work.
I've tried all three. I think I had the least luck with Cialis. Viagra worked well and sometimes not well enough for intercourse. Never used Levitra with a woman, but I am planning on it soon as I've taken it to test it and it has produced great results.

Viagra seems to work the best for me. I found that cialis takes to long to work and erections are not solid rock hard
I will keep my eye on the expiration date, I have never taken one that was out of date but I am glad you mentioned this. How many years are they good for usually?

My Viagra expired almost exactly one year from when I bought it.

Will it depends, mine expires in 14 months so I guess it would be a little over a year. But I only take viagra once a week so when I buy 60 pills they last me forever

This study on rats says, "Combining the centrally acting drug yohimbine with the peripheral conditioner sildenafil might be an approach to erectile dysfunction cases in which sildenafil alone failed."
I couldn't find a study on humans, but they think since it works on rats, it might work on us too.
